Hesse (another German state) also used to attend. I think Belgium compete as two separates because they don't really get on together (I remember the same issue blighting Belgium's JWOC team in the past).
The match started in 1995 in Belgium, with help from a grant for the promotion of international competition from the Foundation for Sports and the Arts, and was in some way compensation for the lack of Sports Council funding for English teams for home internationals. Whilst that grant money ran out after 3-4 years, the competition has continued.
As to why England only go - presumably because nobody else has asked to go, but that's a guess.
