What do clubs do in this respect? My club allows people to withdraw their entry online (and receive a full refund in the form of credit) right up until the last start time.

You can’t just “find a better club” though, can you? Maybe if you live near the boundary or are in/around London perhaps. But for most people, it’s not an option and you’d end up spending more on travel than the old entry fee anyway.
I’m still not buying that ‘most/lots’ of clubs are doing EOD. Half my neighbouring clubs aren’t.
That's certainly true of clubs round here - but that helps to push the "see what the weather is doing" crowd into making a decision and reduces map waste if it is pouring or there's another event on we haven't factored in etc.And some that are, both near and far are basically worded to say ‘you’ll be lucky to get a map’ if you don’t pre-enter.
That is also my experience, but some finishers are very possessive of their maps so expecting them to give it up because you won't commit to coming until the last minute seems like the wrong person misses out.Which whilst possibly true, doesn’t reflect that whenever I’ve been at an event and maps have run out, they’ve been recycled thanks to co-operative finishers.

If you don't want to change club, then get involved with the club committee and change it from the inside, or if you really believe there's enough people who share your desire for EOD start a new "EOD club".

Very, very few regional/local events I've been to recently haven't offered EOD. I know that, because my OH has, recovering from a broken leg, been taking one event at a time and entered on the day. IIRC only one event hasn't offered it in our part of the world (mostly YH, but also orienteering in NW and NE).

I've never run a fell race (too hilly for me!). Do you get a map? Is everyone on the same course? How many maps do you print? Do you ever worry about running out? Any risk of running out of SI cards (or is it old school stop watches and honesty that you followed the right route)? (Are there any similar things for fell races - like water, finishers medals, first aid, parking etc where knowing the number of entries would be helpful?)
If i understood correctly you had 15 entries at 4pm on the day of the event. 5 of them didn't come. Then between 6-7pm 38 people turned up and registered? Do you take cash? Do you take cards? (Just thinking of the other thread where £1 in a bucket for parking was creating a modern-day - "who uses cash" issue). Are those numbers normal for Fell Running? Our local O events are pretty much the opposite. Pre-entries close about a week before the event and >75% of people have entered by then (<5% don't show). Late entries remain open until the night before and will usually make up 20% of the field, and then on the day <5% of people turn up for EOD. Late entries and EOD are of course dependent on the number of spare maps. It's guesswork how many are needed and sometimes we run out, sometimes we are left with so many that we've paid more for wasted maps than credit card fees.

You missed the point spectacularly.
And you should definitely run fell races.
